Submitting a death claim

1. Collect information.
  • How did the client die?
  • What is the date of death?
  • Has a death certificate been issued?

2. Call the carrier claim dept.
3. Follow up with the insurer to make sure the claim pays out.

Submitting a claim - Disability/ Chronic & Critical Illness

1. Collect information.
  • What is the diagnosis?
  • What is the date of diagnosis?​​
  • Is the illness a qualifying condition and is it certified by a physician? Details​
​
2. Submit the claim.
  • If the condition qualifies. Submit the claim
  • If the condition does not qualify, don't submit the claim.
  • If you are not sure if it's a qualifying event, call the claims dept.

3. Follow up with the insurer to make sure the claim pays out.
